Non-Medical Case Management
Non-Medical Case Management helps people living with HIV overcome barriers to care by providing support, referrals, and follow-up for medical and non-medical services. These services help individuals stay engaged in care and access the resources they need.
What services may be included?
- Assessment of needs, strengths, and barriers
- A care plan with goals and action steps
- Referrals to services such as Medicaid, pharmacy support, housing, food, transportation, employment, and other community resources
- Follow-up to check whether referrals were completed
- Ongoing support to help remove barriers to care
How does it work?
- An initial assessment is completed to better understand the person’s needs and barriers.
- A care plan is developed with goals that reflect the person’s priorities and next steps.
- Case managers provide referrals, track progress, and help address barriers over time.
- When services are no longer needed, the case may be closed with appropriate documentation.
What are common reasons for a case to close?
- The individual no longer needs the service
- The individual no longer meets eligibility requirements
- The individual is referred to another program that better meets their needs
- The individual relocates or is no longer receiving services in the area
Important Information
- Not every person needs Non-Medical Case Management. It is intended for people who need extra help accessing or staying in care.
- Some individuals may no longer need services once they are able to manage their care and referrals more independently.
- These services should not duplicate Medical Case Management unless both are necessary and clearly documented.
