Support is available for your emotional and social needs. Whether you’re navigating HIV Diagnosis or caring for someone else living with HIV, you don’t have to do this alone.

Medical Case Management helps people living with HIV who have more complex medical or personal challenges stay connected to care. A medical case manager helps coordinate services, support treatment adherence, and connect individuals to the resources they need to stay healthy.

Patient Navigation helps people living with HIV get connected to medical care, medications, benefits, and support services. Navigators help guide individuals through the system and follow up to make sure they successfully connect to care.

Medical Transportation provides non-emergency transportation to help people living with HIV get to medical appointments, pick up prescriptions, and access other essential support services.

Housing Services help people living with HIV find and maintain safe, stable housing. This may include emergency, short-term, or transitional housing support, along with referrals to longer-term housing resources. Housing services are limited based on funding availability.

Food Bank services help people living with HIV access groceries when financial, health, or mobility challenges make it harder to get the food they need.

Emergency Financial Assistance provides short-term help with basic needs during a crisis so people living with HIV can prioritize their health and stay connected to care.

These services provide outpatient treatment and support for people who are dealing with alcohol or drug use. Services may include counseling, treatment planning, medication support, and relapse prevention without requiring an overnight stay.

Medical Nutrition Therapy helps people living with HIV improve their health through nutrition assessment, counseling, and nutrition support provided by a licensed dietitian.

Non-Medical Case Management helps people living with HIV overcome barriers to care by providing support, referrals, and follow-up for medical and non-medical services. These services help individuals stay engaged in care and access the resources they need.
