Patient Navigation
Patient Navigation helps people living with HIV get connected to medical care, medications, benefits, and support services. Navigators help guide individuals through the system and follow up to make sure they successfully connect to care.
Who is this for?
- People living with HIV who need help getting connected to care, benefits, or support services.
- Individuals who may be new to care, returning to care, or having trouble accessing services.
- People who are preparing to return to the community from jail or prison and need help linking to care.
How does it help?
- Connects people to doctors, clinics, and specialty care
- Helps with referrals to support services such as transportation, food assistance, and housing
- Provides information about medication assistance programs, including the Texas HIV Medication Program and other related programs
- Helps people apply for benefits such as Medicaid, Medicare, Marketplace coverage, Social Security, veterans’ benefits, and other assistance programs
What do we do?
- Explain available services and benefits
- Help determine what programs a person may qualify for
- Assist with referrals and applications
- Follow up to make sure services were accessed and connections to care were completed
